Hope Home Care Support has a range of services (components registered with NDIS) for those facing disability related problems in Australia. We offer personalized care including medical/health care as well as regular personal care for loved ones under your care.

Support coordination

Support coordination helps you to make the best use of your support in the plan. Support coordination is a capacity-building support that helps you to:

  • Understand and use your NDIS plan to pursue your goals
  • Connect you with NDIS providers, community, mainstream and other government services
  • Build your confidence and skills to use and coordinate your support.

Participate Community

 People with disabilities can participate in social and community activities under the NDIS. Participants must participate in social, communal, and recreational activities in a centre or community setting to meet daily and weekly goals. These activities help to develop skills, confidence, and the ability to interact with and participate in community activities. Tuition classes, sports coaching, art classes, camps, and vacation activities are just a few examples. Dance, singing, painting, horseback riding, and other universal leisure activities are available to attendees.

Interpret/ translate

We provide interpreter services to participants to assist them in accessing the supports you have funded in your plan. We will schedule a language support interpreter in person or over the phone.
We can assist you in booking a specific interpreter each time you meet with them, such as if you want the same interpreter or prefer a specific gender.

Household Tasks

We help the individual keep his or her home environment clean. This may include performing necessary household tasks or providing assistance if the individual is unable to do so.

  • Cleaning your bath and/or shower after changing your linen
  • Vacuuming and hanging out the laundry
  • Cleaning the Floors
  • Meal Preparation and Ironing
  • Individualized training/skill development in household chores
  • Grass Cutting
  • Snipping Whipper
  • Weeding Garden edging, tree trimming, path and driveway cleaning

Innovative Community Participation

We offer creative community involvement that is both enjoyable and productive. This is what the NDIA licences us to do. Learning, exercise, community involvement, and enjoyable activities are all available. Our day programmes are also customised to your specific interests and goals. Do you have a specific activity or destination in mind? Community Nursing Care

Community nurses at Hope Home Care Support are caring professionals who are highly skilled, culturally sensitive, and eager to assist.
Community nursing care services are available to meet your clinical and personal needs, allowing you to live an independent and healthy life. A qualified nurse can provide community nursing care services in your home or at our Community Clinic, and you can use them on a regular or as-needed basis.

Assist Travel/ Transport

For participants who are unable to use public transportation due to a disability, we offer NDIS Assist Travel Transport. Our transportation assistance providers are qualified, have a valid driver’s licence, and have passed a police background check. Our special transportation arrangements will transport you from one location to another and then back home. Our support staff will arrange safe and convenient transportation services to help you get to various outings, frequent doctor’s appointments, or weekly shopping mall excursions. Our travel/assistance/transportation specialists are available for weekly or daily trips. These transportation options can make getting around easier and safer.

Assist Personal Activities

Our support staff is knowledgeable and courteous, and they will help you with day-to-day tasks. People who are elderly or disabled find it difficult to carry out their daily tasks. We continue to be in charge of such people’s personal hygiene activities, such as showering, oral hygiene, and clothing, among other things.
We can also assist with eating and drinking, getting into and out of bed, toileting, and bowel and bladder management. Furthermore, we guarantee that we will accompany you to your weekly or monthly doctor’s or dentist’s appointments.
